The Community Toolbox
An online resource for planners working to improve the health of a community. With the mission to “Promote community health and development by connecting people, ideas and resources”, The Community Toolbox provides guidance in community-building skills, offers toolkits for various types of projects, and provides troubleshooting support for commonly seen dilemmas. Effective processes for change and models for taking action are also provided. This resource offers the opportunity to connect with experts and community members, and provides links to databases useful in examining best practices.
The AHRQ Health Care Innovations Exchange provides comprehensive resources and guidance for problem solving, improving the quality of health care, and reducing disparities. Users can search for innovations and quality tools by categories such as disease, setting of care, stage of care, and patient population. Innovations can also be researched by state. Links to upcoming events and podcasts are provided along with resources for funding opportunities.
The National Library of Medicine (NLM), National Institutes of Health (NIH), and the U.S. Department of Health and Human Services (HHS), Office of Disease Prevention and Health Promotion, have worked together to develop preformulated search strategies (Structured Evidence Queries) that search high-quality, peer-reviewed scientific literature to identify research evidence for selected Healthy People 2020 objectives. These one-click strategies search PubMed, an NLM database that provides access to millions of citations from MEDLINE, life science and public health journals, and on-line books. PubMed includes links to many sites providing full text articles and other related resources.
Overlap with many SHIP measures and LHIC priorities make this a usable and valuable tool for planners and clinicians.
The World Cafe

Baltimore Neighborhood Indicators Alliance - Jacob France Institute at the University of Maryland Baltimore (BNIA-JFI)
BNIA-JFI is an organization supported by diverse groups committed to promoting, supporting, and helping people make better decisions using accurate, reliable, and accessible data and indicators to improve the quality of life in Baltimore City neighborhoods. BNIA-JFI builds on and coordinates the related work of citywide nonproﬁt organizations, city and state government agencies, neighborhoods, foundations, businesses, and universities to support and strengthen the principle and practice of well informed decision making for change toward strong neighborhoods, improved quality of life, and a thriving city. BNIA-JFI is well known for the Vital Signs reports which feature indicators in the following topic areas as well as Census demographics: • Housing and Community Development • Children and Family Health and Well-Being • Crime and Safety • Workforce and Economic Development • Sanitation • Urban Environment and Transit • Education and Youth • Neighborhood Acton and Sense of Community
The purpose of Community Commons is to create healthy, sustainable living communities. It allows for networking with other people and organizations that are working on similar issues. The Commons include maps and data from over 3000 participating initiatives around the country. Their website allows individuals to create customized maps by utilizing local initiatives and public data. Diagrams, maps, and tools from the Community Commons can help demonstrate the current state of your community, raise awareness, and push common goals. Mobilizing for Action through Planning and Partnerships (MAPP)
A community-driven strategic planning process for improving community health. Facilitated by public health leaders, this framework helps communities apply strategic thinking to prioritize public health issues and identify resources to address them. MAPP is not an agency-focused assessment process; rather, it is an interactive process that can improve the efficiency, effectiveness, and ultimately the performance of local public health systems. A management tool that can facilitate collaboration among human service agencies, as a method of decentralizing services, and as an innovative regulatory process. At a minimum, the term implies that expected results (also known as goals) are clearly articulated, and that data are regularly collected and reported to address questions of whether results have been achieved. RBA can be developed and used at different levels: state, community, agency, or program. The level of the goals dictates responsibility for achievement of results.
