Starr Metrick - Effective January 19, 2001 - Revocation

Alice Abrash Effective April 25, 2003 - License granted with conditions.  License surrendered on October 24, 2005.

Yolanda Romero - Effective July 18, 2003 - Revocation

Sabrina Burgess - Effective May 21, 2004 - Suspension for 30 days and probation for 1 year.  Probation terminated May 21, 2005.  Ms. Burgess' license is in good standing.

Sandra Goscinski - Effective July 15, 2005 - Reprimand

Mitchell R. Fox - Effective December 15, 2006 - Probation. All conditions were met and probation terminated December 15, 2007. Mr. Fox's license is in good standing.

Kimberly L. Locati-Boucher, OTR/L - Effective February 16, 2007 - Six-month suspension (stayed) and 2-year probationary period effective November 17, 2006.

Paul Duane Phillips, OTA - Effective April 3, 2007 - Letter of Surrender.

Elizabeth A. Flanigan, OT - Effective May 8, 2007 - Summary Suspension. (Ms. Flanigan will not appear on active or expired status listing.)

Elizabeth A. Flanigan, OT - Effective June 6, 2007 - Summary Suspension lifted and placed on Probation for two years subject to conditions.  All conditions were met and probation terminated June 6, 2009. Ms. Flanigan's license is in good standing.

Valerie Dejean, OT - Effective January 2, 2008 - Letter of Surrender

Sheri Present, OT - Effective December 1, 2007 - 1-year suspension (all but 3 months stayed) and 2-year probation period effective November 16, 2007.(pages 22 - 33 of consent order)

Maria Rupp, OT - Effective January 16, 2008 - 2-year suspension (all but 60 days stayed) with conditions followed by a 2-year probation with conditions. All conditions were met and probation terminated January 16, 2010. Ms. Rupp's license is in good standing.

Kimberly L. Boucher, OTR/L - Effective April 6, 2009 - Suspension of 3 years (all but 1 year stayed) followed by a 3-year probation with conditions. Suspension was lifted on July 27, 2010.  Probation is still in effect.

Marnie J. Czarnecki, OTR/L - Effective April 23, 2009 - Letter of Surrender

Laura B. Cox - Effective June 22, 2009 - Probation for one year.  All conditions were met and probation terminated on September 16, 2011.  Ms. Cox's license is in good standing.

Laura Lubin - Effective September 18, 2009 - Suspension for 3 months (all stayed); Probation for two years with conditions.  All conditions were met and probation terminated on September 18, 2011.  Ms. Lubin's license is in good standing. 

Sheri Present - Effective January 1, 2010 - Suspension for 9 months and probation for an additional year (October 1, 2010 - October 1, 2011).

Sandra Severin - Effective July 29, 2010 - Letter of Surrender

Sheri Present - Effective May 18, 2011 - Revocation

Frank W. McCreary - Effective November 28, 2011 - Probation for 2 years with conditions

Kimberly A. Wyatt - Effective February 17, 2012 - Revocation

Anjuli Kelley - Effective February 29, 2012 - Probation.  Probation terminated March 15, 2013.  Ms. Kelley's license is in good standing.

Persis Appiagyei - Effective February 27, 2013 - Suspension for 1 year and probation for 3 years (February 27, 2013 - February 27, 2016)
