Reinstatement of an Expired License
For audiologists and speech-language pathologists:
An individual whose license expired on or after May 31, 2010 may reinstate their license. Call 410-764-4725 to request a reinstatement application.
An individual whose licenses expired on or before May 31, 2008 must reapply and meet all current requirements in effect. Use the licensure application documents found on the forms tab.
For hearing aid dispensers:
An individual whose license expired on or after January 31, 2009 may reinstate their license. Call 410-764-4725 to request a reinstatement application.
An individual whose license expired on or before January 31, 2007 must reapply and meet all current requirements in effect. Use the licensure application documents found on the forms tab.
Changes to HAD Examination Process
The examination process for limited licensees in hearing aid dispensing will change during Summer 2013. The Board will not administer the Written Examination. The Written Examination will be administered as a computer-based examination by a third-party at a testing site approved by the International Hearing Society.
The Practical Examination will still be administered by the Board. The Practical Examination will be offered up to 10 times each calendar year.
Information regarding these changes has been e-mailed to all limited licenses in hearing aid dispensing and all full licensees in hearing aid dispensing and audiology that are eligible to be a supervisor of a limited licensee in hearing aid dispensing. More information will be posted to the Board's website as it becomes available.

Procedure to Request Verfication of Licensure
A type-written request must be submitted to the Board. The request must include the licensee’s name, profession, and license number. The request must indicate the Board or other entity to which the verification is to be sent.
Fee:
There is no fee assessed for a verification letter if the individual making the request is an active licensee with a license issued by this Board.
If the individual making the request does not hold an active license issued by this Board the licensee must submit a fee of $25.00 for the verification letter. Verification fees are non-refundable.
An individual that holds a current licensee may submit the request by one of the following three methods:

An individual who does not hold a current license must submit the written request via mail along with the $25.00 fee to the Ms. Campbell’s attention as indicated above. |
